For activities that require a Liability Form, every participant must have a current FBM medical and liability waiver on file. You can check your Church Center profile to see when yours expires. If a new one is needed, download the appropriate liability form after and bring it to the church at the times listed by event when a notary will be available. Each Liability Form NEEDS to be signed in the presence of a notary.
